- Collaborates effectively with clinical team members in preventive care, and treatment of acute and chronic illness.

- Obtains, performs, records, and maintains patient histories and physical examinations.
- Evaluates and treat acute and chronic illnesses and injuries.
- Performs or orders appropriate laboratory tests, x-rays, and other tests.
- Obtains consultation or referrals for patient services not provided by the clinic.
- Provides health promotion services, including school physicals, immunization clinics, well-child clinics, well-woman clinics, adult focus clinics, blood pressure screening clinics, nutritional counseling, and family planning counseling.
- Provides additional services determined by agreement with Residency Medical Director.
- Adheres to team agreements and assists in any revisions as necessary.
- Provides emergency services to patients when necessary and in accordance with La Clinica’s policies and procedures and generally accepted standards of care. All clinical services shall be provided in accordance with La Clinica’s Scope of Project for purposes of coverage under the Federal Tort Claims Act.
- Sees patients at the primary clinic and other sites as assigned by La Clinica.
- Completes and maintains all privileging/credentialing requirements and standards established by La Clinica.
- Participates in non-clinical duties as requested, such as, but not limited to, meetings, peer review, quality improvement activities, and medical care-related policy/procedure/protocol development. Cooperates with La Clinica’s quality assurance and utilization review protocols
- Responds to the recommendations for new or modified approaches to patient treatment recommended by peer review and utilization review processes.Complies with all contractual requirements regarding credentialing, utilization management, quality assurance, grievance procedures, provider directory listings, and any other policies and procedures that such contracts may require.
- Maintains the confidentiality of patients by complying with HIPAA policies.
- Completes assigned training via La Clinica’s learning module system (Relias) by the required due date.
- Continues medical education as necessary to meet requirements of licensure by the Oregon State Board of Nursing or Oregon Medical Board and to maintain competency in the practice of family medicine and hospital privileges.

We are searching for experienced, dynamic, advanced practice providers to join one of our primary care sites. As a primary care medical home, we serve 28,000 patients annually throughout our six health care centers, 34-chair dental clinic, and mobile health center—a 40-foot converted RV. We are an innovative non-profit organization focused on providing exceptional health care to all who have barriers to care. We include integrative behavioral and dental health care in our practices.
At all our primary care sites, our multi-disciplinary teams focus on providing excellent medical care and supporting our patients' emotional and social needs, both adults and children. These teams include a mix of physicians, family nurse practitioners, physician assistants, integrated behavioral health teams, nurses, MAs, registration personnel, and team leads. Each team has a clinical leader, and our practice also employs a psychiatric nurse practitioner and a child and adolescent psychiatrist. Our primary care sites provide outpatient services with no inpatient or obstetrical responsibilities.

Join a collaborative and supportive team at La Clinica. We are looking for a full-time behavioral health clinician to join our fast-paced, patient-centered primary care team.
We are dedicated to the primary care behavioral health (PCBH) integration model and support early learner-integrated behavioral health clinicians (IBHC) by offering:
- 3.5 hrs/month group and individual clinical supervision, which meets licensing board requirements
- Opportunities for experiential didactics/case conferences with a PCBH focus
- The Integrated Behavioral Health Clinician (IBHC) is a vital part of our primary care team at La Clinica. The IBHC works closely with primary care providers, nurses, medical assistants, and other team members to address the mental health and substance use challenges of our patient population. Our IBHCs provide evidence-based, targeted assessment, consultation, and therapy services to promote behavioral health across the lifespan. This includes easy-access, interdisciplinary care for children, teens, adults, and families, both individually and in group settings, making referrals when necessary.
Key Responsibilities:
- Collaborative care: Work closely with La Clinica medical and dental providers to deliver comprehensive clinical services that target interventions aimed at improving patient quality of life and clinical outcomes.
- Health care provider communication: Address referral concerns from health care providers by delivering focused, clear feedback in both written and verbal forms.
- Health assessment: Identify and evaluate health risk behaviors, assess and diagnose mental health and substance use conditions, develop care plans, and provide brief, targeted interventions to support optimal mental, behavioral, emotional, and physical health.
- Stepped care participation: Collaborate with psychiatric providers for psychiatric medication management needs and refer patients to specialty mental health and substance use care as necessary.
- Risk intervention: Support the team in assessing and intervening for patients at risk of harming themselves or others and address situations involving domestic violence by identifying safety plans and referring to appropriate services.
- Team involvement: Participate actively in patient care staffing with the clinical care team to ensure comprehensive patient care.
- Educational contribution: Provide clinical consultation and psycho-educational programs on behavioral health-related topics to staff, students, consumers, and other agencies as needed.
- Continuous improvement: Participate in clinic and performance improvement meetings and in-service training opportunities to stay updated and contribute to ongoing clinic advancements.
- Supervisory role: You may provide clinical supervision to unlicensed behavioral health clinicians and master’s degree students, fostering growth and development within the team.

We are looking for an experienced pediatric nurse practitioner to work in our West Medford Health Center and our school-based sites.
At all our primary care sites, our multi-disciplinary teams focus on providing excellent medical care and supporting our patients' emotional and social needs. These teams include a mix of physicians, family nurse practitioners, physician assistants, integrated behavioral health teams, nurses, MAs, registration personnel, and team leads. Each team has a clinical leader, and our practice also employs a psychiatric nurse practitioner and a child and adolescent psychiatrist. Our primary care sites provide outpatient services with no inpatient responsibilities.
At all our school-based sites, our multidisciplinary teams focus on providing excellent medical and behavioral health care and supporting the emotional and social needs of youth. The provider's goals at school-based sites include:

Responsibilities include:
- Provide paperwork and information, including verbal instructions on how to complete forms, to patients
- Accurately collect confidential, sensitive patient information used for federal grant reporting
- Enter data from completed paperwork into a computer database for charting and reporting
- Accurately assign payor source to each qualifying patient visit
- Assess all incoming patients toward determining eligibility for healthcare grants and services
- Check-in patients for scheduled appointments
- Coordinate with clinic staff to determine availability for non-scheduled care and make sure patients are seen promptly
- Perform cash handling responsibilities, including preparing receipts, collecting money for services, and preparing daily deposit
- Order and maintain inventory of forms and supplies
- Create and update forms, type and prepare correspondence, and maintain front office desk manual
- Follow workflows to capture activities to meet patient engagement standards as outlined in the APCM program
- Conduct outreach efforts to make sure patients stay connected to OHP (Oregon Health Plan) and La Clinica services
- Other duties as assigned
Education and/or experience:
- Bilingual/bi-literate skills in English and Spanish required
- Minimum: High school diploma or GED
- Six months of work in a medical office setting or one year of experience in a high-volume direct public contact setting
- BLS healthcare provider (basic life support certification)
- Excellent interpersonal communication and problem-solving skills
- Skills to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions
- Skilled at using electronic health records as applicable to the area of work (EPIC)
Preferred:
- Basic knowledge of adult learning models

Although some of our staff work in clinical positions that require a healthcare background, we have many entry-level positions and support employees in positions such as quality, data, IT, human resources, and administration that do not require previous work in health care. Our team can train you for your work in entry-level positions such as at our reception desks.

As a healthcare organization, we strongly encourage everyone to get vaccinated for their safety and that of our patients. While we no longer mandate COVID vaccination for employment, we still highly encourage it. Employees who choose not to be vaccinated must complete a declination form. We require masking during flu season for unvaccinated employees and anticipate requiring the same for COVID-19 during peak transmission.
