Medical schedulers are available Mondays-Fridays from 8 a.m. to 5 p.m.

For dental appointments at any of our centers, call 541-512-3900.
Dental schedulers are available Mondays-Fridays from 7:30 a.m. to 6 p.m.

We require a 24-hour cancellation notice on appointments to allow us time to accommodate demand from other patients.

La Clinica makes every effort to honor scheduled appointment times but emergencies do arise. Your wait time may vary.

WHAT TO BRING TO YOUR APPOINTMENT

Please bring identification, any insurance cards you have, and a list of medications you are taking. This helps us provide you better service and process your insurance more quickly.

You may also want to complete a release of information form before your visit. This form allows us to request your records from another healthcare provider’s office. There is also a form to use when you want us to share your healthcare information with others (for example, a parent or child).
